1. Complete these forms:

     a. CM-010 Civil Cover Sheet

     b. NC-200 Petition for Change of Name, Recognition of Change of Gender, and Issuance of New Birth Certificate. Beginning September 1, 2018, the options for gender marker are male, female, and nonbinary. 

     c. NC-110 Attachment to NC-200

     d. NC-125/NC-225 Order to Show Cause for Change of Name to Conform to Gender Identity

     e. NC-230 Decree Changing Name and Order Recognizing Change of Gender and for Issuance of New Birth Certificate

2. Make Copies of Forms.

3. File Your Forms:

     a. File the forms in the Superior Court for the county where you live.

     b. Pay the filing fee or apply for a fee waiver (FW-001). For more information about fee waivers, see FW-001 INFO

4. Court Approval: 

     a. Beginning September 1, 2018, the name and gender change application will be granted without a hearing unless someone files a timely, good cause objection.

     b. If the judge signs form NC-230, obtain a certified copy from the court.
